SCCCW Convention - Request for 3 Honors and Memorials

Please fill out this form to request three Honors and/or Memorials for someone special while donating to the South Carolina Council of Catholic Women (SCCCW). If you would like a different quantity, other forms are available on the SCCCW website, scccw.org. Under Activities & Events, choose Convention.

The Board of Directors of SCCCW has established an Honors and Memorials Program to give affiliates an opportunity to honor or remember someone in a special way. Names of the persons submitted will be presented during a portion of the SCCCW Convention and published in a program booklet. A $10.00 donation is suggested for each name submitted.

To request a different number of Honors and/or Memorials, please use the specific form for the number you wish to submit.  The forms may be found on the SCCCW website, scccw.org. Under Activities & Events, choose Convention.

Choose either Honor or Memorial, the name of the person being honored or memorialized, and indicate if an acknowledgement of this honor or memorial should be sent to a specific person or not.

If you would like SCCCW to send an acknowledgement of the first Honor or Memorial, please provide the name of the person to whom the acknowledgement should be sent along with their full mailing address or email address.
 
 
 
2nd Honor/Memorial
To submit a second Honor or Memorial, choose which you would like and enter the information.
